I will provide you with a superbill that you may submit directly to your insurance provider. I cannot guarantee the coverage of your insurance benefits or reimbursement.

If you call your insurance provider to inquire about receiving reimbursement for nutrition services, it may be helpful to ask these questions:

  • Do I have out-of-network benefits for nutrition services provided by a Registered Dietitian?

  • Which nutrition-related diagnostic codes does my plan cover? (ask specifically about the diagnosis code Z71.3)

  • Does my plan cover CPT codes 97802, 97803?

  • Does my plan cover telehealth services?

  • What percentage of the service fee should I expect to be reimbursed for?

  • Is pre-authorization required to submit a claim?

  • How much of the deductible have I met?
